如欲索取免費門票，請填妥這表格，於 11 月 9 日起交回指定通利琴行，或以電郵辨理手續。
網上索取門票方法：
1. 免費門票由 11 月 9 日 (星期五) 早上 9 時起接受電郵登記 (在此時間前，電郵地址並未開啟)。每人只可索取門票一次，每人兩票。門票有限，派完即止。
2. 請將姓名、電話及地址，電郵至 ticket@hkpo-sus.com。資料不全，將不獲處理。
3. 成功索取門票者，將收到電郵確認。我們將於確認後七個工作天內郵寄門票至你提供的地址，請確保閣下之地址完整無誤。
4. 成功索取門票者，將可參加大抽獎，獎品包括精美紀念品及港樂音樂會門票。得獎者將獲個別通知。
